Common Application Forms

• Students who are applying to colleges and universities that accept the Common Application should apply online. For more information on the Common Application, to apply to Common App schools, and to access all the Common App forms, please visit the Common App website.

Attached below, please find:

• The NHS Release Form, used by the Guidance Office for seniors applying to college.
• The Transcript Request Form. Click here for more information on the transcript request procedure.
• The Student Data Sheet, which juniors are asked to fill out and return to their guidance counselors as part of the college application process. Guidance counselors will use the information provided by the student, as well as other information from parents and teachers, to inform their discussions with students around post-graduation planning, and to help them as they write a counselor recommendation for colleges.
• The Junior Parent Questionnaire, which can be downloaded and printed. Junior parents are asked to fill out the questionnaire and return it to their student's guidance counselor before the end of the school year. Counselors will be compiling and summarizing information in order to complete a counselor recommendation in the fall, and parents' perspectives can be very helpful in this process.
